EBSC Participation Levy Claim Form

Please fill in this form to submit your claim for hours worked towards your participation levy commitment. Each family must submit this form to claim for time worked for the swim club. Your postdated cheques will be returned upon completion of the hours worked as per your swimmer's level (see below.)

There are two times when you may submit your claim, these dates are:

Submission date by:

1. Dec. ?, 2007

2. May 26th, 2008

RULES/REGULATIONS

1.	Levy hours should be submitted through the on-line forms only; and submitted only when requested to do so.
2.	Please submit whatever hours you have accumulated to the date of the submission.
3.	In the first submission, if you have submitted one-half or more than one-half of your levy commitment, then your first cheque will be returned to you by December 31st of that year.
4.	If you have not reached “one-half” of your levy commitment regardless of hours worked, your first cheque will be cashed.
5.	If you have reached all of your levy commitment by the first submission, both cheques will be returned to you.
6.	For those who need to hand in the second submission (near the end of May), you may do so after recieving an email notification . For others that need a little bit more time, the last date would be mid-June. Please don’t leave your levy hours until the last month, as you may be out of luck.
7.	Additional time cannot be carried over to the next swim season.
8.	You will not be paid for any time worked over and above your commitment.
